REGISTER AS A NEW PATIENT?
Heath House accepts everyone who moves into the practice area and wishes to register.
To register, please call in to reception with your medical card, if you have it, along with some official identification eg passport, driving licence etc. You will be required to complete a small registration form and invited to attend a registration medical with one of our practice nurses or our health care assistant.
Although patients are registered with the 'Practice' and not an individual doctor, patients can request to see a regular doctor. Please advise a receptionist if you wish to have a preferred doctor recorded. Whenever you require an appointment the surgery will try and provide this with your preferred doctor. However, this may not always be possible.
This medical will only take about 15 minutes and ensures that the practice has all the relevant information regarding your health care needs. A more detailed form will be given to you to complete at home, and should be returned to the nurse when you attend your medical appointment.
If you are not entitled to NHS treatment you may be able to register with the practice as a private patient. Please ask at reception for more details.
